Forms
- Registration Related Forms
- Consortium Registration - Follow this step-by-step guide to completing the request to enroll in a Consortium course at another institution.
- GSEHD Combined Enrollment Application Form - Complete if you are currently in a GSEHD Master's program and would like to dual enroll in a GSEHD certificate.
- Half-Time/Full-Time Status Certification - For students who intend to register for less than the number of credit hours required for approved purposes and who need to be certified as in full or half-time status
- Leave of Absence/Continuous Enrollment Form - Request a leave of absence from your degree program or request continuous enrollment status if requirements are met
- Complete Withdrawal Form - Withdraw as a student of GSEHD/GW
- Program Plan of Study Change Request (MA,EdS,Certificate) - Request a substitution for different courses in place of program requirements.
- Incomplete Grade Contract - If you receive a grade of "I" or incomplete, you and your instructor may agree on a new timeframe for completion.
- Registration Transaction - May be used if students are unable to add a course(s) through GWeb for various reasons; Learn more about when to use this form >
- Doctoral Student Forms
- Doctoral Student Checklist - For reference only, this checklist serves as your overarching to-do list during your doctoral program.
- Dissertation Committee Membership - Formally indicate the members of your committee.
- Dissertation Proposal Approval Form - Your faculty will use this form to approve your dissertation proposal.
- Dissertation Approval Form - Your faculty will use this form to approve your dissertation as ready for the oral defense.
- Request for Dissertation Oral Examination - Use this form to formally submit your dissertation materials and select the date for your oral defense.
- ETD Approval Form - Your faculty will use this form to approve the final, written version of your dissertation for ProQuest submission and graduation.
- Program Plan of Study Change Request (EdD, PhD) - Request a substitution for different courses in place of program requirements.
- Graduation/Post-Graduation Related Forms
- Online Graduation Application Instructions - Access step-by-step directions for applying to graduate using the online form available on GWeb.
- Late (Paper) Graduation Application - Only to be completed when instructed to by Student Success Coach
- Program Verification for Licensure - Graduates that have completed one of GSEHD’s state-approved educator preparation programs can request verification for licensure/employment purposes.
- Other
- Portfolio Completion - To be completed by program advisor signifying completion of portfolio
- Comprehensive Examination Request Special Sitting - If unable to complete the comprehensive exam on the scheduled date/s, a request can be submitted for a rescheduled date.
- GW Student Health Insurance Waiver Form - All on-campus students are automatically charged and enrolled in the GW Student Health Insurance Plan (SHIP). If you have external insurance, complete this form to waive the GW SHIP.
NOTE: Any form requiring a Dean’s Office or School Official Signature should be returned to your Student Success Coach for processing.

The Dissertation Support Group (DSG) provides doctoral students a repository of materials to refer to while writing a dissertation. Each semester the DSG hosts several workshops designed to assist students in the research and writing process, as well as online support sessions where doctoral students, faculty, and alumni provide mutual support.
